AT&T is requesting approval of a specific use permit for telecommunications
towers, antennas, and ancillary buildings to allow the construction of an unmanned
telecommunications facility and the placement of six (6) panel antennas on an
existing Oncor transmission tower. The site is privately owned and located at 1200
N. Kimball Avenue.

• The proposed new site was designed to provide improved coverage
between Hwy 114 and Dove Road.
• A Low Coverage 'hole' exists just north of Carroll Middle School,
which results in poor call quality.
• Both Data and Voice traffic in this area has increased to the point that
call quality is further diminished and data throughput is below
acceptable levels.
• Our new site will take over coverage in this area and provide much
improved voice quality and higher data rates for our customers.

(naturally grown) that have grown around the base of the tower. They will eventually be removed
for utility line clearance.

Public Works Engineering
The following comments are based on the review of plans received on: 8/31/2012
It is the applicant's responsibility to contact the department representative shown above and
make modifications as required by the comment.
GENERAL COMMENTS:
1. A ROW permit shall be obtained from the Public Works Operations Department for connections with the
City's sewer, water or storm drain inlets. Contact Public Works Operations at (817) 748-8082.

2. Label the proposed right-of-way for the proposed street.
3. Show the curb return radius for the proposed drive. The curb return radius for driveways must be 30'
minimum. All fire lane radii must be 30' minimum.
4. Sidewalk widths must conform to the Southlake Pathways Plan.
Use the City of Southlake GPS monuments whenever possible.
EASEMENTS:
Water and sewer lines cannot cross property lines without being in a public easement or in the ROW. All
waterlines in easements or ROW must be constructed to City standards.
DRAINAGE COMMENTS:
1. Driveway culvert will need to be sized by a professional engineer to pass a 100-year storm event.
18" RCP Minimum is required.
2. Provide a grading plan which includes drainage arrows and 2' contours. Contours may be obtained from
the City of Southlake.
All storm sewers collecting runoff from the public street shall be RCP.
The discharge of post development runoff must have no adverse impact on downstream properties and
meet the provisions of Ordinance # 605.
ENGINEERING INFORMATIONAL COMMENTS:
Submit 4 copies of the civil construction plans (22" x 34" full size sheets) and a completed Construction
Plan Checklist as part of the first submittal for review directly to the Public Works Administration
Department. The plans shall conform to the most recent construction plan checklist, standard details and
general notes which are located on the City's website.

Any hazardous waste being discharged must be pretreated, Ordinance No. 836.
This review is preliminary. Additional requirements may be necessary with the review of
construction plans.

GENERAL COMMENTS:
Provide a portable fire extinguisher, a minimum of 5 pound, ABC multipurpose for the interior of
the structure.

FIRE LANE COMMENTS:
Fire apparatus access needs to be provided within 150 feet of all exterior portions of the
perimeter of the building on a "hose -lay" basis for an un-sprinkled building. Fire apparatus
access needs to be an all-weather surface, asphalt or concrete, 24 feet wide and able to support
the imposed loads of fire apparatus.
FIRE INFORMATIONAL COMMENTS:
All commercial buildings are required to have Knox Box rapid entry systems installed.
Boxes can be ordered at www.knoxbox.com or contact the Fire Marshal's Office.
General Informational Comments:
Prior to permitting, provide a certificate of insurance to the city, issued by an insurance company
licensed to do business in the state of Texas indicating that the applicant carries comprehensive
general liability insurance with limits of liability thereunder of not less than: bodily injury: $500,000
for injury to any one person and $1,000,000 for all injuries sustained by more than one person in
any occurrence; property damage: $1,000,000 for damage as a result of any one accident. The
applicant shall provide the city with a renewal certificate within then (10) business days of each
renewal. Any insurance required to be provided by the applicant herein may be provided by a
blanket insurance policy covering this property and other locations occupied by the applicant,
provided such blanket insurance policy complies with all of the other requirements as to the type
and amount of insurance required. The applicant may also fulfill the requirements under this
section through a program of self-insurance, subject to approval by the city, which approval shall
not be unreasonably withheld. If the applicant elects to self -insure, then the applicant shall
furnish the city with a letter stating that there is a self-insurance program in effect that provides for
the same, or greater, coverage than required of the applicant herein. The applicant agrees to
furnish the city with certificate of insurance certifying that the applicant has in force and affect the
above specified insurance. The certificate and renewal certificates shall provide that insurance
shall not be canceled or changed unless 30 days' prior written notice is just given to the city.
* The applicant should be aware that prior to issuance of a building permit a Plat must be
processed and filed in the County Plat Records, a fully corrected site plan, landscape plan,
irrigation plan, and building plans, must be submitted for approval and all required fees must be
paid. This may include but not be limited to the following fees: Park Fee, Perimeter Street Fee,
Water & Sewer Impact and Tap Fees, and related Permit Fees.
Each backhaul provider shall be identified and have all necessary franchises, permits, and
certificates. The identity of other providers who co -locate to the site and their backhaul providers
shall be provided as well.
* All mechanical equipment must be screened of view right-of-ways and residential properties in
accordance with the Zoning Ordinance No. 480, as amended.
* All lighting must comply with the Lighting Ordinance No. 693, as amended.
* All development must comply with the Drainage Ordinance No. 605 and the Erosion and
Sediment Control Ordinance No. 946, as amended.

No lettering, symbols, images, or trademarks large enough to be legible to occupants of vehicular
traffic on any adjacent roadway shall be placed on, or affixed to, any part of a telecommunications
tower, platform, antenna or ancillary structure.
* Development must comply with permitted building color(s). Please refer to Zoning Ordinance No.
480, as amended, Section 33 for building color regulations.
* In the event that disruption of the landscaping or facility occurs due to its location within the
existing utility easement, mitigation will be required.

A RESOLUTION OF THE CITY COUNCIL OF THE CITY OF SOUTHLAKE,
TEXAS, GRANTING A SPECIFIC USE PERMIT FOR
TELECOMMUNICATION TOWER, ANTENNAS AND ANCILLARY
BUILDINGS WITHIN THE CITY OF SOUTHLAKE, TEXAS, LOCATED AT
1200 NORTH KIMBALL AVENUE, SOUTHLAKE, TARRANT COUNTY,
TEXAS, BEING DESCRIBED AS TRACT 2A03, FRANCIS THROOP
SURVEY, ABSTRACT NO. 1511, SOUTHLAKE, TARRANT COUNTY,
TEXAS AS DEPICTED ON THE APPROVED SITE EXHIBIT ATTACHED
HERETO AND INCORPORATED HEREIN AS EXHIBIT "A" AND
PROVIDING AN EFFECTIVE DATE.
WHEREAS, a Specific Use Permit for a telecommunication tower, antennas and ancillary
buildings has been requested by a person or corporation having a proprietary interest in the property
zoned as "B-1" Business Service Park District; and,
WHEREAS, in accordance with the requirements of Section 45.7 of the City's Comprehensive
Zoning Ordinance, the Planning and Zoning Commission and the City Council have given the requisite
notices by publication and otherwise, and have afforded the persons interested and situated in the
affected area and in the vicinity thereof; and,
WHEREAS, the City Council does hereby find and determine that the granting of such Specific
Use Permit is in the best interest of the public health, safety, morals and general welfare of the City.
NOW, THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF
SOUTHLAKE, TEXAS:
SECTION 1.
A Specific Use Permit is hereby granted for telecommunication tower, antennas and ancillary buildings,
on property within the City of Southlake, Texas, located at 1200 North Kimball Avenue, Southlake,
Texas, location being described as Tract 2A03, Francis Throop Survey, Abstract No. 1511, Southlake,
Tarrant County, Texas, more fully and completely described in Exhibit "A", and as depicted on the
approved Site Plan attached hereto and incorporated herein as Exhibit "B" and providing an effective
date, subject to the provisions contained in the comprehensive zoning ordinance and the restrictions set
forth herein. The following specific requirements and special conditions shall be applicable to the

SECTION 2.
This resolution shall become effective on the date of approval by the City Council.
