PROJECT: Master Sign Plan Amendment for Forest Park Medical Center
EXECUTIVE
SUMMARY: On behalf of Forest Park Medical Center Southlake, 3d Signs & Graphics is
requesting approval of an amendment to the approved Master Sign Plan for the
new Forest Park Medical Center located at 335 East State Highway 114 to include
attached signage on the parking garage.
REQUEST: The proposed Master Sign Plan amendment includes sign regulations for attached
signs on the north and northeast elevations of the parking garage. The original
sign package did not include any signage for the parking garage other than
address numerals.

PROPERTY SITUATION: The property is located at 335 E. State Highway 114
LEGAL DESCRIPTION: Lots 1 — 5, Block 1, Southlake Medical District
HISTORY: March 5, 2013; City Council approved (7-0) the Master Sign Plan as
presented to include the revised Emergency Monument sign that
was distributed during the meeting. (MSP13-002)
The original sign package did not include any signage for the
parking garage other than address numerals.

REQUEST: The applicant is requesting approval of an amendment to the
