The requested action for this item is to approve a commercial developer
agreement with Southlake Land Holdings, LP for the Southlake Regional
Medical Center Addition which will be known as the Forest Park Medical
Center at Southlake.

The Southlake Regional Medical Center Addition is located at 335 SH 114 along the
eastbound frontage road.

The Southlake Regional Medical Center Addition Phase I construction will consist
of building a Hospital, Parking Garage and a Medical Office Building. The hospital
will have a minimum square footage of 130,000. The Medical Office Building will
have a minimum square footage of 60,000. The total construction value for Phase I
is $50 million dollars.

The City of Southlake is entering into an agreement with Southlake Land Holdings,
LP because they are requesting variances from the standard fees and park
dedication ordinances typically required with commercial development. The
prerequisite for waiving the park dedication will require that the construction of the
site includes a saved tree grouping along an area adjacent to SH 114. Also, the City
requests that several water features be constructed, including one at the north
entrance to the hospital.
With the future construction of Phase II, an updated commercial developer
agreement will be required and will include that additional right of way on E.
Highland Street be dedicated to accommodate a left turn lane into and out of the
site. The current traffic impact analysis justifies that the expansion of the site will
require a turn lane.

Southlake Land Holdings, LP is requesting a 50% fee reduction for the
administrative and construction inspection fees, zoning and platting fees, building
permit and inspection fees and the public works plan review and inspection fees.
The fee reductions will commence upon the issuance of the first building permit and
last for a period of 5 years.

The City of Southlake agrees to reimburse Southlake Land Holdings, LP for the
construction of 1130 linear feet of 12” water line and 730 linear feet of 12” sanitary
sewer line along the eastbound frontage road of SH 114. The fees for the design
and construction of the 12” sanitary sewer line shall not exceed $80,000. The City
will acquire all necessary easement and/or right of way needed for the construction
of the off-site sanitary sewer line. The water line and sewer line will be constructed
concurrently with the project.

The map above shows the locations ofthe proposed 12” water line and 12” sanitary
sewer line along the eastbound frontage road of SH 114. The illustration also
shows the off-site easement needed for the 12” sanitary sewer line which the City
will acquire as part of this agreement.

The reimbursement for the construction of the 12” water line and 12” sanitary sewer
line will come from credits to the water impact fee and wastewater impact fee due
with Phase I construction as described in Section 4.A.1 in the 380 Agreement. Also,
two payments, each being fifty percent (50%) of the total reimbursement amount
described above less the credits will be paid. The first payment to be in the first City
Fiscal Year after the issuance of the first Certificate of Occupancy, and the second
payment to be in the second City Fiscal year after the issuance of the first
Certificate of Occupancy.
Future funding for this project will be included in the budget request for the Capital
Improvements Projects Utility Fund.

Approval of this commercial developer agreement links this project to the City’s
strategy map relative to the focus areas of collaborate with select partners to
implement service solutions and invest to provide and maintain high quality public
assets.
