SUBJECT: Approval of Change Order No. 5 to CMPA, Inc. for the Department of
Public Safety — West Facility — City of Southlake Project No. 00 -3320.
Action Requested: Approve Change Order No. 5 to CMPA, Inc.
Background
Information: The City of Southlake entered into an agreement with CMPA, Inc. for the
construction of the Department of Public Safety — West Facility on
December 12, 2000. Change Order No. 5 is necessitated as a result of the
following issues:
Item #1 — Landscapinz Modifications
In the early stages of the project, staff requested a proposal from CMPA to
make six changes to the Landscaping Plan for the facility. After receiving the
proposal from CMPA, the decision was made to accept only two of the items;
1) replacing one Dwarf Holly with a Carissa Holly at no additional cost and 2)
replacing the specified Weathermatic irrigation control panel with a Rainbird
irrigation control panel, for equipment consistency reasons, for an increase in
cost of $4,174.50.
PSA and staff have reviewed this proposal and find it to be acceptable. It is
recommended that this change be approved for an increase of $4,174.50.
Item #2 — Interior Gas Line Modifications
In Change Order No. 4, it was explained that the gas service for building
would be moved from the front of the building to the back of the building.
This change necessitated changes to the interior gas piping, as well. However,
CMPA proposed to make these changes for no change in price. This item is,
therefore, accepted for no change in price.

Item #3 Additional 3 / " Backflow Assembly
During installation of the plumbing for the building, it was determined that an
additional 3 /4" backflow preventer assembly was required in the area of the
sally port in order to meet code requirements. Staff requested a proposal from
CMPA to provide the additional backflow preventer. CMPA proposed to
provide the backflow preventer for an increase of $405.14.
PSA and staff have reviewed this proposal and find it to be acceptable. It is
recommended that this change be approved for an increase of $405.14.
Item #4 — HVAC Duct Revisions
Minor issues have been identified by the PSA that required minor changes
pertaining to the HVAC ducting in the building. Staff requested a proposal
from CMPA to make these changes. CMPA proposed to make these changes
for no change in price. Therefore, this item is approved for no change in
price.
Item #5 — Additional Phone/Data Lines
The City's Technical Services Department requested installation of two
additional phone /data lines in order to allow communications and data transfer
between all City facilities. CMPA proposed to provide the additional lines for
$121.00.
PSA and staff have reviewed this proposal and find it to be acceptable. It is
recommended that this change be approved for an increase of $121.00.
Item #6 — Likht Fixture /Ceilinz Framinz Conflict
An RFI from CMPA identified a conflict between lighting fixtures and the
ceiling framing.
Staff, PSA and CMPA identified a solution to the conflict that could be
accomplished at no additional cost. Therefore, this item is approved for
no change in price.

Item #7 - Ventilation Steel Rid ,-e Plates
During installation of the HVAC systems, it was determined that, though
ridge vents had been installed in the roof ridges, the steel ridge plates
located under the roof ridge vents had not been vented. Without the ridge
plates being vented, the roof ridge vents can not operate properly and can
not effectively vent the building's attic space. Ventilation of the attic
space is preferred in order to reduce heating and cooling loads and
moisture accumulation in this space.
Staff requested a proposal from CMPA to ventilate the ridge plates after
they had been installed. CMPA proposed to vent the ridge plates for an
increase of $1,293.00.
PSA and staff have reviewed this proposal and find it to be acceptable. It
is recommended that this change be approved for an increase of $1,293.00.
Financial
Considerations: The Contract currently provides a "Contingency" item in the amount of
$150,000.00. If approved, these changes can be included under this
"Contingency" item and no change in the Contract Amount or the Contract
Time will occur as a result of these changes. The balance of the Contingency
item is as follows:
