Item 4A1The Tolycystic Fidney Disease Foundation is devoted to improving clinical
treatment and discovering a cure for Polycystic Yidney Disease; and,
Wfiiereas, Tolyycystic Yidney Disease is the most common genetic, fife threatening disease affecting
more than 600, 000Americans and an estimated 12.5 million people worldwide — regardless of
gender, age, race or ethnic origin, and,
Whereas, the City of Southlafie, Teas, has an interest in the development and provision of high
quality services to children, youth andfamilies who sufferfrom this disease. The citizens from the
City of Southlake will come together and recognize Tolycystic Kidney Disease Awareness .Month
during the month ofSeptem6er, and,
Whereas, during Tolycystic Yidney Disease Awareness month and beyond, the City of Southfa�e,
families, citizens, religious congregations, schools, community -based organizations, businesses,
political and culturalgroups will join together to recognize and further their commitment to reach a
cure for those affected 6y Tolycystic Kinney Disease; and,
Wfiiereas, this commitment must reflect support and responsibility at the local lever beginning with
individualfamilies andcommunities, to ensure continuedimprovement in the quality of all citizen's
lives; and,
Now, Tfierefore, I, Andy Wam 6sganss, 94ayor of the City of Southfake, on 6ehalf of the City
Council, do hereby proclaim September, 2005 as

in Southlake, Teas, and urge all citizens to join in this observance.
