City of Southlake, Texas

Re : Spine Team Texas Surgery
Zoning Change - Miron Drive
Dear Mr. Killough
We are requesting an amendment/zoning change to the Miron Drive site Lot 4R
Block 1. The ST-2 generalized site plan district with 0-1 Office District use needs
to be modified to include "ambulatory surgical center /clinic (no over night stay)" as
a permitted use. AIso the covered short term parking space ( "carport ") needs to be
included on the plan attached to the zoning as a permitted accessory use
The existing approved site plan remains as built with two additions requested to
Building 6. We request to add a canopy over an existing parking space. The
canopy is designed to match the style of the existing canopy over the door. We
request to add a masonry screen wall around our mechanical equipment. The brick
material and detailing will match the existing building.
Our submittal includes 12 copies of the following:

Proposed Building Use Narrative
Miron Drive #4
Building 6 on original Site Plan Submittal
Spine Team Texas is excited about our plans to establish an ambulatory surgery center on the Miron Drive
site. We currently operate an ambulatory surgery center and a medical practice at 1545 E Southlake Blvd. in
the medical office building next to Harris Southlake Hospital. Due to the growth of our medical practice, we
have outgrown our current ambulatory surgery center. Therefore, we have selected the Miron Drive site as
an excellent location to expand our ambulatory surgery center. We will continue to operate our medical
practice in the medical office building on Southlake Blvd. In our ambulatory surgery center we perform
epidural and cervical steroid injections for our patients. We will have approximately 15 employees working in
our new center, and will treat an average of 20 patients per day. Patients will stay at the center an average of 2
hours in order to have their procedure and adequately recover. We will typically operate the center from 7am
to 5 :30 Monday through Friday.
The original approved site plan identified building 6 as medical use and allocated parking for that use. The
revised plan will not change the number of approved parking spaces.
