PROJECT: Master Sign Plan for The Carlyle at Stonebridge Park
REQUEST:
GHLA, Inc., on behalf of First Carlyle Capital Funding, LLC, is requesting approval
of a Master Sign Plan for The Carlyle at Stonebridge Park. The Carlyle is a skilled
nursing facility owned and operated by Cantex Senior Communities and located at
170 Stonebridge Lane. The Master Sign Plan includes sign regulations for a wall
sign, monument sign, and therapy entry directional sign.

REQUEST:
The applicant is requesting approval of a Master Sign Plan. A
summary of the request follows:
Wall Sign
– One sign, pin mounted metal letters as shown on the
attached Exhibit A. This sign will be the initial identifier of the
development as you approach from Southlake Blvd. The signage
will be illuminated by a ground mounted flood light.

Monument Sign
– One sign, a brick masonry sign constructed in
the style and materials of the building, as illustrated on the attached
Exhibit B. This sign will have a one side face containing the logo,
name and address of the facility. The sign will be illuminated by a
ground mounted flood light.

Therapy Entry Directional Sign
– One sign, a molded plastic
frame with a blade sign consistent with the style of the building, as
illustrated on the attached Exhibit C. This sign identifies the entry to
the Therapy Center. The size of the sign is 3 feet 2 inches wide by
3 feet 6 inches tall. The posts are 5-inches square. The Blade sign
will be 24” x 24”, white with black letters reading “Blueridge
Rehabilitation Center”
