A. Recognition of former Senior Advisory Commission Chair Charlie Boyd — Mr.
Boyd submitted his resignation from the Senior Advisory Commission (SAC)
and his positon as chairman, effective May 10, 2016. City Council appointed
Mr. Boyd to the Senior Advisory Commission on October 10, 2006. Charlie
has been a strong participant in SAC and an advocate for senior issues for the
last decade and provided strong leadership and direction for SAC as he
served as Vice Chair August 12, 2014 — November 10, 2015, and then as
Chairman until his recent resignation on May 10, 2016.
3. Agenda Item No. 3. Public Forum: — This is the public's opportunity to address the
Commission about non -agenda items. During Public Forum, the Commission will not
discuss, consider, or take action on any item presented. The Commissions' options
during this topic are 1) to listen, 2) ask questions and respond to the presenter only,
3) request staff to look into the issue and report back to the Commission and/or 4)
request that the Chair put the item on a future agenda for the Commission to discuss
or consider.

C. Southlake Senior Funding, Inc. — This item is a standing item on the agenda to
allow for a report on Southlake Senior Funding, Inc., (SSFI). This item has no
packet document.
D. City Manager's Office — This is a standing item on the agenda to allow for an
update from the City's Assistant to the City Manager Daniel Cortez. This item
has no packet document.
E. Community Services — This is a standing item on the agenda to allow for an
update from the Community Services Department staff regarding current and
future developments.
This month's staff update includes the May Senior Center Membership Report,
which is included in your packet, the unveiling of the Senior Memorial Wall in
the Dick McCauley Library, and the creation of an information binder regarding
the Senior Advisory Commission.
F. Senior Services — This is an opportunity for the Senior Services Coordinator to
present information about existing and future programs and activities. This
item has no packet document.

this item is placed on the agenda to allow the Commission an opportunity to
elect a vice -chair if the position of Chair is filled by the current vice -chair, if the

C. Recommendation on Fiscal Year 2017 Senior Services General Fund Budget
— Staff will present the FY 2017 Senior Services Budget at the meeting on

1. CALL TO ORDER
Chairman Boyd called the meeting of the Senior Advisory Commission to order at
1:07 p.m.
Chair Boyd announced his resignation from the Senior Advisory Commission
effective this meeting due to his impending move from Southlake.
2. PUBLIC FORUM — Chairman Boyd opened public forum. There being no one to
speak, Chairman Boyd closed public forum.
3. REPORTS
3A. METROPORT MEALS ON WHEELS
MMOW Executive Director Mary King presented the March MMOW Report and was
available to answer questions. MMOW and City staff will hold quarterly meetings to
address any issues or concerns. Solutions to the concerns discussed at the April
meeting have been resolved by staff and MMOW.

CARS board representative Brighton Greene summarized the ridership information in
the March 2016 CARS Report. Ridership is on pace to exceed last year's numbers. A
proclamation to recognize CARS volunteers will be presented at the annual CARS
Drivers Appreciation Dinner on May 12, 2016. Mr. Greene was available to answer
questions.
3C. SOUTHLAKE SENIOR FUNDING, INC.
There was no Southlake Senior Funding, Inc. report in the absence of SSFI
representative Bernice McCauley.
3D. CITY MANAGER'S OFFICE
Pilar Schank will no longer attend the SAC meetings this year due to additional duties
she has been given, but Assistant to the City Manager Daniel Cortez will represent
the City Manager's Office at the meetings.
Assistant to the City Manager Daniel Cortez informed the Commission about Verizon
FIOS (transitioned to Frontier Communications) Internet, cable, phone service
concerns. Contact Frontier Communications regarding any service issues at 1-800-
921-8101 or the City Manager's Office.
3E. COMMUNITY SERVICES
The Community Services Report included the following two topics:
Membership Report
Deputy Director Kate Meacham provided the April Membership Report, highlighting
the growth in attendance and membership at the Senior Activity Center. Ms.
Meacham was available to respond to questions.
Site Plan for The Mara Southlake Phase 2 (Chamaions Club
Director Tribble shared presentations to update the Board about the site plans for
The Marq Southlake Phase 2 (Champions Club) and the Southlake Tennis Center
that will be presented at the SPIN Forum this evening. Staff responded to questions
from the Board.
The Commission discussed options to manage entry from the senior's wellness room
to the rest of the athletic amenities with the issue being how to regulate entry to fee -
based membership level for the athletic amenities in Champions Club. Commission
members offered suggestions. Staff will continue to seek a solution.
The Commission discussed use of the senior fitness room as a dedicated senior
fitness room and options the Commission may wish to explore in the future.

Membership scenarios were discussed and staff will continue to look into different
scenarios, including a care -giver arrangement fee rate membership.
Chair Charlie Boyd announced his resignation from the Commission and left the
meeting at this time. Vice Chair Beverly Blake continued to chair the meeting.
3F. SENIOR SERVICES COORDINATOR
Soheila Phelps provided a brief report about membership at the Senior Activity
Center and attendance for various classes. An increase in field trips was reported.
Class location information is available on the monitors and outside classrooms in
Legends Hall to help patrons find classes and programs. Ms. Phelps responded to
questions from the Commission.
4. DISCUSSION
Deputy Director Kate Meacham reviewed a draft of the survey that will be published
May 16-May 27 to active users of the facility to gauge membership satisfaction with
Legends Hall, the current senior programming, and the staff, and requested feedback
from the Commission. The Commission did not have any changes to the draft survey.
The results will be shared with the Senior Advisory Commission at an upcoming
meeting.
4113. LOCAL GOVERNMENT OFFICE CONFLICTS DISCLOSURE STATEMENT
Deputy Director Meacham presented information and State requirements concerning
the Local Government Officer Government Conflicts Disclosure Statement that all
City employees and appointed boards, commissions and committees members may
be subject to if they or a family member have employment or other business
relationships as described in the Statement with any existing City vendor. Contact the
Office of the City Secretary for more information. Director Tribble and Deputy Director
Meacham responded to questions.
5. CONSENT AGENDA Consent Agenda items are considered to be routine by the
Commission and are enacted with one motion.

Deputy Director Meacham reviewed the final design of the senior memorial exhibit for
the back wall in the Dick McCauley Library and was available to answer questions
from the Commission. One member expressed concern a listing of all the deceased
members was morbid. There was no further discussion by the Commission.

SUBJECT: Election of Chairman for Senior Advisory Commission and Possible Election of
a Vice Chair
Action Requested: Election of a Chairman for the Senior Advisory Commission (SAC) to fill
the unexpired term of the officer position of chairman left by the
resignation of Charlie Boyd; and the possible election of a Vice Chair if
the Chair position is filled by the current Chair.
Background
Information: With the resignation of Chair Charlie Boyd, effective at the May 10,
2016 SAC meeting, this item is placed on the agenda to allow the
Commission an opportunity to elect a chair to fill the unexpired term left
by Mr. Boyd's resignation. This item will also provide the Commission
an opportunity to elect a Vice Chair if the position of Chair is filled by
the current Vice Chair, if the Commission so desires.
The remaining term for the Chair and Vice Chair positions will end when
new officers are elected following City Council appointments in October
2017. The current slate of officers (Chair Charlie Boyd, Vice Chair
Beverly Blake and Secretary Marge Kyle) were elected November 10,
2015 for a two year term, November 2015 — October 2017.
The Senior Advisory Commission Bylaws Article III states regarding the
process for election of officers in Section 3.1(d): A vacancy in the office
of any elected officer shall be filled by a vote of the majority of the
commission members at the next regular scheduled meeting, following
the effective date of the vacancy."
As set out in the Bylaws, the responsibilities of the Chairman are:
• To set the agenda for meeting;
• Preside at all meetings of the Commission;
• Perform all duties incident to the office of Chairman including the
appointment of all committee and subcommittees of the
Commission; and
• Such other duties as may be prescribed by the Commission and
the City Council.

OF THE
CITY OF SOUTHLAKE SENIOR ADVISORY COMMISSION
ARTICLE I
PURPOSE AND POWERS
Section 1.1 Purpose
The Southlake City Council established the Southlake Senior Advisory Commission to act as an advisory board to
the City Council in all matters pertaining to senior programs and issues.
Section 1.2 Powers
In fulfillment of its purpose the Southlake Senior Advisory commission shall be governed by and shall have all the
powers set forth in the Southlake City Code and other applicable law.
ARTICLE II
COMMISSION MEMBERS
Section 2.1 Number and Terms of Office
(a) The Commission shall consist of seven (7) residents of the City of Southlake, Texas and shall be appointed
by the City Council.
(b) Members shall be appointed for two-year terms. Members shall serve without compensation. All
appointments for regular and/or alternate members to fill unexpired terms shall be filled in the same
manner as regular appointments and shall terminate as per the original term being filled.
Section 2.2 Meetines of the Commission
The Commission shall meet regularly the second Tuesday of each month. Members not able to attend a regular
meeting or special meeting shall notify the Community Services staff liaison or the Chairman of the Commission at
least two hours prior to the posted meeting. Any member with (3) consecutive unexcused absences from regular
meetings shall be reported to the City Council, which may at its discretion, remove the commission member and
appoint a replacement. All meetings and deliberations of the commission shall be called, convened, held, and
conducted, and notice shall be given to the public, in accordance with Chapter 551 of the Texas Local Government
Code.
Section 2.3 Special Meetines
Upon the request of any Commission member, the Chairman shall call a special meeting of the Commission. The
Chairman shall fix a place and time for such special meetings of the Commission subject to posting public notice in
compliance with Chapter 551 of the Texas Local Government Code.
Section 2.4 Ouorum
Four (4) members of the Commission shall constitute a quorum to conduct official business of the Commission.
Section 2.5 Conduct of Business
(a) At the meeting of the Commission, matters pertaining to the business of the Commission, as posted on the
agenda, shall be considered in accordance with the rules of procedure as from time to time prescribed by
the Commission.

(b) At all meetings of the Commission the chair shall preside. In the absence of the chair, the Vice chair shall
preside.
(c) The Chair shall be a voting member of the Commission.
(d) Minutes of the Commission meeting shall be filed with the City Secretary and distributed to members of
the City Council.
Section 2.6 Ex-officio Members
The commission may, as it deems appropriate, appoint ex-officio members to the Commission if such is determined
to be in the best interest of the Commission. Consideration shall be given to appointing individual ex-officio
members or individuals who represent organizations whose purpose is to promote the health, safety, and well being
of seniors. Such ex-officio members shall have no voting authority for senior advisory matters. The terms for ex-
officio members will be at the discretion of the Commission.
ARTICLE III
OFFICERS
Section 3.1 Officers and Terms of Office
(a) The Officers of the Commission shall be a chairman, vice-chairman and secretary.
(b) At the first regular meeting following the annual appointment of new commission members by the city
council, the commission shall elect by majority vote from the members, a chair, vice -chair, and secretary,
each to serve two year terms or until their successors are elected.
(c) Any officer shall be subject to removal from office at any time by a vote of the majority of the
Commission.
(d) A vacancy in the office of any elected officer shall be filled by a vote of the majority of the commission
members at the next regular scheduled meeting, following the effective date of the vacancy.
Section 3.2 Powers of the Chairman
The Chairman shall set the agenda for and preside at all meetings of the Commission. The Chairman shall perform
all duties incident to the office of Chairman including the appointment of all committees and subcommittees of the
Commission and such other duties as may be prescribed by the Commission and the City Council.
Section 3.3 Vice -Chairman
In the absence of the Chairman or in the event of his/her inability or refusal to act, the Vice -Chairman shall perform
the duties of the Chairman, and when so acting shall have all the powers of and be subject to all the restrictions upon
the Chairman. The Vice -Chair shall perform such other duties as from time to time may be assigned to him/her by
the Chairman of the Commission.
Section 3.4 Secretar-y
The Secretary shall ensure that the minutes of each Commission meeting shall be filed with the City Secretary and in
general shall perform all other duties as from time to time may be assigned to him/her by the Chairman of the
Commission. In the absence of the Chairman and Vice -Chairman or in the event of their inability or refusal to act,
the Secretary shall perform the duties of the Chairman, and when so acting shall have all the powers of and be
subject to all the restrictions upon the Chairman. In the absence of the Secretary, the Chairman or acting Chairman
shall designate a Commission member to perform the duties of Secretary.

Section 3.5 Compensation
Officers who are members of the Commission shall not receive any salary or consideration for their services.
ARTICLE IV
ROLE AND RESPONSIBILITIES OF THE SENIOR ADVISORY COMMISSION
Section 4.1 Roles of the Senior Advisory Commission
(a) Act in an advisory capacity to the City Council in all matters pertaining to senior programs and issues
affecting senior citizens.
(b) Recommend policies to improve senior services and improvements.
(c) Recommend senior related programs for development of recreational facilities, areas and improved
recreation services.
(d) Review and consider recommendations from the Southlake Seniors and other senior organizations regarding
senior related programs and activities. Develop recommendations to the City Council regarding programs
that have funding, safety, or facility implications.
(e) Submit recommendations to the City Council regarding contractual agreements that impact senior reiated
services, including, but not limited to, the congregate lunch program and home delivered meal program and
transportation services for seniors.
(f) Prepare and submit to the City Council an annual report of senior related programs and services. Such
review shall be submitted in May, prior to the election of new officers.
(g) Review and provide input on budget items related to senior services, activities, and programs.

The Marq is a new center and it attracts new comers like me, who are not always aware of all the activities that are
taking place. A simple solution is to put up a daily activity board by the entrance, so everyone can see whats going on
that day. I'd love to see the center run in its full capacity.

Staff to provide/make the coffee for the Bridge group. It is very inconvenient for us to make it in the room & then bring

Listen to what seniors want, not just what the manager wants. Change the day for the senior lunch to Tuesday,
Thursday or Friday so more people can benefit from it.
Pay attention to the recommendations the seniors make instead of ignoring them.

Enjoy the plays -more if possible. Staff is great -at least 2 out of 3. Building is beautiful, just not cozy or functional for
older seniors. People are friendly, helpful, caring.

The staff is very patient & friendly in assisting the needs of the senior adults. they are the BEST! An I can't say

1 really enjoy the zumba class. Rosie, the instructor is wonderful. The music, pace and regularly updated new dance
moves make this class a joy to attend. Another one is the Tai Chi class.

Some arts and crafts geared to both male & female - trips in local areas. JK & Mus. art district Ft Worth, trip to
speedway - horse racing etc.
Tai Chi - Good for balance. Good for Senior.
Tai Chi Excellent instruction and encouragement.
The fitness programs are excellent. I am also looking forward to some of the educational programs.
