ITEM 3b - STATEMENT OF QUALIFICATIONS ARTICLE 1: REQUIREMENTS FOR THE STATEMENT OF QUALIFICATIONS 1.01 The Statement of Qualifications must be submitted and include, as a minimum, the information as described herein. Failure to submit the required information in the Statement of Qualifications may result in the Owner considering the Proposal non -responsive and result in rejection of the Proposal by the Owner. Proposers may be required to provide supplemental information if requested by the Owner to clarify, enhance or supplement the information provided in the Statement of Qualifications. 1.02 Proposers must provide the information requested in the Statement of Qualifications using the forms attached to this Section. A copy of these forms can be provided in Microsoft Word to assist with the preparation of the Statement of Qualifications. Information in these forms must be provided completely and in detail. The information in these forms will be used to make direct comparisons with the information provided by other Proposers. Failure to include the information completely and clearly may result in lower scores in the evaluations. Information that cannot be totally incorporated in the form may be included in an appendix to the form. This appendix must be clearly referenced by appendix number in the form, and the appended material must include the appendix number on every sheet of the appendix. The appendix must include only the information that responds to the question or item number,to which the appended information applies. ARTICLE 2: STATEMENT OF QUALIFICATIONS SUBMITTAL REQUIREMENTS, EVALUATION CRITERIA AND WEIGHTING STATEMENT OF QUALIFICATIONS SUBMITTAL REQUIREMENTS ARE AS FOLLOWS: A. Proposer General Information: 1. The proposer shall complete and submit Table 1 and any needed appendix to the form (See section 1.02 above). B. Project Organization and Experience and Qualifications of Proposed Key Personnel: 1. Provide an organizational chart for this Project showing the structure of Proposer's Project team with lines identifying the significant positions which shall include as a minimum the Key Personnel positions and other positions that are responsible for major elements of the Project execution. Significant positions indicated on the organizational chart can have named individuals other than Key Personnel. For those additional individuals, a summary paragraph can be included on these individual's qualifications and experience to describe their Project role and responsibilities. Except for the Key Personnel, it is acceptable for chart to have a position without a named individual. The chart shall indicate the anticipated percentage of each person's time that will be committed to the Project. At a minimum the anticipated percentage of each Key Personnel's time will be provided. Critical support elements of project management and administration, quality control, safety, subcontractor management, etc. shall be identified. Charts and accompanying summary paragraph on other individuals named on the charts will be not subject to the page limit. 2. The Proposer shall complete and submit Table 2. Provide the names of the Key Personnel that will be actively working on this Project in Table 2. Key Personnel include the Project Manager, Project Superintendent, Safety Manager and Quality Control Manager. If one or more of these Key Personnel roles are to be filled by one individual, this information is to be provided with the list of proposed individuals. The Proposer must provide the services of the proposed Key Personnel for the life of the Project as a condition of qualification. Failure to provide the proposed Key Personnel may result in the disqualification of the Proposer and may void the award of the Contract. Key Personnel may only be changed with the prior written consent of the Owner. Pertinent work history which describes project experience along with role and responsibilities on those projects d. Information on prior projects that demonstrates experience with similar projects as this Project 4. Proposers are to include a list of the current project assignments for each of the Key Personnel proposed, the anticipated completion date for these assignments and the percentage of the time they will have available to devote to the City of Southlake Project. The Project Superintendent must be dedicated to the City of Southlake Project full time for the duration of the Project. 5. The Proposer will provide information on past projects where Key Personnel worked together. C. Project Resources; Subcontractors and Suppliers; and Safety 1. She will be responsible for managing and administering the Safety and Health Program for the entire site and overseeing project wide training and awarenes consistent with the current activities. 13 SOUTHLAKE Brandon Martindale Superintendent Aaron James T AWAW Samantha Juarez Sr. Project Manager At r,I Safetv Man z I Aeron McCarty Ouality Control COMMITMENT To Quality, Community, and Our Clients Auger Water Treatment Plant Upgrades General Project Description: This Project consisted of expanding the existing Auger Water Treatment Plant to a capacity of 12 MGD and includes the addition of plate settlers in the existing sedimentation basins, addition of two low-pressure membrane units, addition of one strainer, and addition of one vertical turbine pump and associated valves and piping at the existing membrane feed pump station. Challenges and Opportunities for Success Due to the high demand for water in the City of Midlothian, the membrane units were fast tracked as a pre -selected item from DuPont/ EvoquaTechnologies. There was a project milestone that needed to be met in order for the City of produce enough water to keep up with the demand. Felix worked nights to keep the schedule on track due to the multiple plant shutdowns that were required to install the membrane units. This allowed the Owner to not see any disruption in plant production during the installations. Felix was able to deliver on that milestone and completed the installation of the two membrane units along with the new strainer prior to the deadline. Felix maintained and successfully implemented a strategic safety plan through coordination with their internal safety department. Felix's safety department maintains written policies and procedures which protect employees, City staff, and all other personnel on -site. No accidents occurred throughout the fast -track construction duration and through multiple critical facility shutdowns. During the winter months, Felix took advantage of the plant's Lower flow season to install the plate settlers in the basins. The plate settlers were able to improve flow for the City in both basins as the existing units had become outdated and not functioning properly. Once instaLled and tested, the City was extremely happy with the results as they saw significant improvements in the water quality through production. Near the end of the project, Felix was tasked with shutting down the whole plant and installing a new PLC system for the City. This included a whole plant shutdown for two weeks. Water was diverted from the plant to a neighboring plant to allow the new system to be implemented. Felix worked hand in hand with Freese and Nichols along with the City of Midlothian through the entire process and the implementation was successful. RELEVANT PROJECT EXPERIENCE Robinson Road Pump Station General Project Description: The Robinson Road Pump Station is a critical infrastructure project designed to enhance the water distribution capacity for the city. This project involved the construction of a new 13.3 million gallons per day (MGD) water pump station with provisions for future expansion to 20 MGD. The pump station features three horizontal split -case pumps, with an additional open stall reserved for future growth. Key elements of this project included the installation of a standby generator and approximately 650 linear feet of 30-inch yard piping to replace the city's existing pump station. The project also encompassed necessary electrical and site work, as well as a high -end architectural facade that integrates the station into the city's vision for the busy corridor where it is located. The facade features premium materials such as brick, porcelain tile, and phenolic rainscreen panels, reflecting the city's commitment to enhancing the aesthetic appeal of its infrastructure. The pump station is designed with a drive -through bay accessible via two overhead doors, ensuring ease of maintenance and operational efficiency. A concrete trench within the building routes cables from the electrical room to the pump room, adding complexity to the foundation's concrete work but ensuring a robust and reliable electrical system. This project is not only a significant upgrade to the city's water infrastructure but also a statement of the city's dedication to both functionality and design excellence. RELEVANT PROJECT EXPERIENCE Keller Alta Vista BPS General Project Description: The scope of work includes construction of a new 18 MGD pump station, demolition of the existing pump station and the associated construction phasing and sequencing to maintain existing operations. Includes purchase and installation of four new horizontal centrifugal pumps, generator, electrical gear and yard piping / civil upgrades. The City of Keller used Freese and Nichols as its procurement advisor. Being able to deliver water by April 1, 2019 was critical and provided the greatest risk. Felix Construction Company was chosen using a two-step CMAR procurement process. At this point, the Felix functioned as a General Contractor consultant by providing preconstruction services during the progression of the project design. The delivery team came together for informal partnering after Felix was on -board which established the cornerstone for the project's culture. In early November 2018, the GM P proposal was approved by Council and the construction project was off to the races. The existing pump demolition began in earnest and power was cut to the existing pump station. From that point there was no turning back. Throughout the project, the water demands were monitored. Heavier than normal rains helped keep the shutdown window open longer than expected. May 1, 2019, became the target date to have water pumping out of the new facility. Pump tests were performed and water was actually pumping into the City's system by May 15, 2019. The City was able to keep up with demands in the system during the entire Winter/Spring shutdown and was in position to supply demands going into the Summer 2019 peak demand period. The project has been a success. Felix was able to deliver the project on time and under budget. Challenges and Opportunities for Success One of the major questions during the preconstruction service phase was whether a temporary bypass pumping contingency would be needed at the station should the construction window close prior to the new pump station's ability to pump water. It was invaluable to have a CMAR on board during this discussion to help weigh the risks associated with the demolition of an existing facility and to use the practical knowledge that comes from a builder that has worked on similar projects involving replacement of facilities during tight shutdown windows. A significant measure of the project's success was how natural the team culture aligned with the partnering expectations. The CMAR method allowed for day-to-day delivery of the project in a manner that was mostly "invisible" to City management and customers. This CMAR experience has inspired confidence with City management that alternative project delivery methods are worthy. With the many challenges of an extremely tight site and a tight construction window, the CMAR alternative delivery method proved its worth and is recommended for projects like this one in the future. RELEVANT PROJECT EXPERIENCE Celina Pump Station Improvements General Project Description: Felix was awarded the City of Celina Pump Station Improvements (successful bidder). The project consisted of the removal and replacement of existing pumps, associated electrical equipment, piping, and demolition of a clear well. Major construction consisted of installation of vertical and horizontal split case pumps, piping, pre-engineered metal building, chemical feed system, new VFD's, backup generators, and roadway improvements. Challenges and Opportunities for Success In the face of unprecedented growth, the City of Celina hired Garver in 2017 to provide engineering consulting services for a city-wide water master plan update. Felix Construction was the successful low bidder on the fast - track design and construction project, NTP was not provided until late January 2018. However, Felix was prepared to mobilize the week that the NTP was issued. To expedite the project Felix and Garver began the review of submittals prior to the NTP starting in December 2017. The fast -track design and construction schedule required all team members to remain in constant communication. The project required certain milestones completion dates,135 days following 10 days from the NTP for pump #4 at Celina Road Pump Station and 210 days from NTP for the entire project. Felix provided regular updates to the City and Garver and mobilized quickly with Long -lead item submittals that were delivered immediately upon notice to proceed. Felix Construction maintained and successfully implemented a strategic safety plan through coordination with their internal safety department. Felix's safety department maintains written policies and procedures which protect employees, City staff, and all other personnel on -site. No accidents occurred throughout the fast -track construction duration and through multiple critical facility shutdowns. The City of Celina has one take point for the city's water supply. Due to the accelerated timeline and critical need to protect the water source, multiple steps were taken to minimize down time and service shutdowns during construction. For this reason, the possibility of adverse conditions were identified early and coordinated between all parties involved in order to prepare appropriately. Multiple rain days during the beginning of construction and the need for the site to remain operational are two examples where this project appropriately worked through adverse conditions. Both sites needed to remain operational at all times. During extended periods of heavy rain, indoor tasks were completed instead of stopping work. In some cases, work that could be continued in the rain was completed to keep the project moving forward to meet the client needs. When shutdowns were needed to place critical pieces of infrastructure, shutdown requests were submitted at least 14 days ahead of time to allow the City to prepare. In preparation of shutdowns, all piping and valves were laid out and connected ahead of time to minimize delays. Detailed coordination was completed between the City staff, Felix, and Garver. With any construction project, it faces challenges. Felix and Garver faced a variety of challenges on this short window project but worked as a team and the project went online precisely in time to meet project demand. I - - - ;/ RELEVANT PROJECT EXPERIENCE CITY OF SANGER: WASTEWATER TREATMENT PLANT General Project Description: This project included construction of a new raw wastewater pump station, a new peak flow storage basin, modifications to the existing bar screen structure, modifications to package plant 1 through 4, modifications to the existing blower building, construction of two new final clarifiers, a new return activated sludge pump station, a new ultraviolet disinfection facility, a new effluent flow measurement structure, a new solids dewatering facility, a new electrical building, relocation of an existing electric generator and connections to existing motor control center, and other miscellaneous plant modifications at the Sanger Wastewater Treatment Plant. Challenges and Opportunities for Success This project was a conventional hard bid project, but Felix s team along with Allen Plummer were able to value engineer some items in the beginning of the project in order to help meet the City of Sanger's budget for the project. Felix and the City of Sanger expected some additional change orders throughout the project due to some major unknowns. In order for Felix to take down existing plants 1, 3, & 4 and evaluate the overall condition of them we had to complete our contract portion of the project first. Once we completed our original portion of the project we were instructed by the owner to take down plants 1, 3, & 4 and it was determined each plant needed structural steel repairs, full coatings to the interior and exterior, and new catwalks to each tank. RELEVANT PROJECT EXPERIENCE CITY OF MANSFIELD: BUD ERVIN WTP CLEARWELL NO.4 General Project Description: This project was the construction of a 3.5 MG cast -in -place concrete clear -well at the Bud Ervin Water Treatment Plant. It also involved the implementation of surge control systems for the Lower Pressure Plan High Service Pump Station, along with the installation of a new vertical turbine pump. The scope encompassed various critical aspects, including the integration of piping, valves, electrical systems, and instrumentation upgrades. Notably, the initiative also included a substantial Time and Materials (T&M) effort for the replacement of tube settlers, adding to the project's comprehensive improvements. The project operations were in close proximity to the school which necessitated beginning work prior to the start of school traffic and concluding either before or after its peak. A strong emphasis was placed on maintaining good relations with neighbors, resulting in a notable absence of complaints. Start times were adjusted to minimize noise disruption and demonstrate consideration for nearby residents. Additionally, a church situated opposite the plant provided a valuable collaborative opportunity. Felix cultivated a positive rapport, securing permission to utilize surplus area for a laydown yard, facilitating material storage. The prospect of re -accessing this space was also factored into the project's considerations. Challenges and Opportunities for Success: The primary hurdle encountered involved the site's constrained space, necessitating the exclusive utilization of a single crane. Meticulous timing was of utmost importance, especially concerning the laying of pipes externally.
