BACKGROUND INFORMATION
OWNER: Edward Moayyad
APPLICANT: Blake Architects
PURPOSE: Replace the existing church structure with a two story medical office
building.
PROPERTY SITUATION: 190 S. White Chapel Boulevard
LEGAL DESCRIPTION: Lot 4, H. Granberry No. 581 Addition
LAND USE CATEGORY: Public/Semi Public
CURRENT ZONING: “CS” Community Service District
REQUESTED ZONING: “O-1” Office District
HISTORY: A special use permit for a church use was approved with the passage of
Ordinance No. 261-44 in December of 1982.
A plat showing for Lot 4, Hiram Granberry No. 581 Addition was approved
in February 1994 and filed in the County in March of 1994.
The “CS” community service district zoning was placed on the property with
the adoption of the Comprehensive Zoning Ordinance No. 480 and the
corresponding official zoning map in September of 1989.
TRANSPORTATION
ASSESSMENT: Master Thoroughfare Plan
The Master Thoroughfare Plan recommends S. White Chapel Boulevard to be
a 5-lane, undivided arterial street with 94’ of right-of-way. The right-of-way
dedicated by the plat was based on the 84’ right-of-way 5 lane divided
arterial which was planned at that time.
Existing Area Road Network and Conditions
The proposed office will have one driveway intersecting with S. White
Chapel Blvd.
S. White Chapel is currently a 2-lane, undivided roadway. The segment at
the intersection with Southlake Blvd has been widened to 5-lanes with a
middle left-turn lane. No plans for near future improvement have been made.

PATHWAYS MASTER
PLAN: A 6 foot wide concrete trail is planned along the west side of S. White Chapel
Blvd. and is shown on the site plan.
WATER & SEWER: A 12” water line currently exists along the east side of S. White Chapel. The
applicant is proposed to connect to the sanitary sewer line extended by the
Stratfort Parc residential development to the south.
STAFF COMMENTS: Attached is Site Plan Review Summary No. 3, dated April 30, 2004. The following
variance is being requested:
• Setback: A minimum setback of 4:1 slope setback is required from single
family residential property. The applicant is requesting a setback of 15’
feet.
• Articulation: The east and west facades do not meet the horizontal
articulation requirements.
Please note that the zoning proposed is not compliant with the City’s Master
Land Use Plan’s “Public/Semi-public” Land Use Designation on the subject
area. A justification letter from the applicant is included as part of
Attachment ‘C’. Any motion for approval of this item should include
justification for non-compliance with the Land Use Plan.

OFFER THE FOLLOWING STIPULATIONS. THESE STIPULATIONS ARE HEREBY MADE CONDITIONS OF SITE PLAN

1. Correct the south building line to be determined by a 4:1 slop. This will require the building to be
setback ±120’ from the south property line. Per Section 43.11, “no non-residential building may
encroach in the area above a line having a slope of 4:1 from any property line of a residentially zoned
property with a low or medium density residential land use designation in the Comprehensive Land
Use Plan.” The applicant proposes a 15 feet setback. (A variance has been requested.)
2. Provide horizontal articulation on the east and west building facades. A maximum wall length of 36
feet with a minimum off-set of 2 feet is required. A wall length of 57 feet with no off-set is
proposed. (A variance has been requested.)
3. Correct the measure stacking depth to measure from the east property line to the beginning of the
drive heading south. The measured distance is approximately 70’.
INFORMATIONAL COMMENTS
* Lighting must comply with the Outdoor Lighting Ordinance No. 693-B.
* No review of proposed signs is intended with this site plan. A separate building permit is required
prior to construction of any signs.
* The applicant should be aware that prior to issuance of a building permit a fully corrected site plan,
landscape plan, irrigation plan, and building plans, must be submitted for approval and all required
fees must be paid. This may include but not be limited to the following fees: Park Fee, Perimeter
Street Fee, Water & Sewer Impact and Tap Fees, and related Permit Fees.
