• City of Southlake, Texas MEMORANDUM July 18, 1997 TO: Honorable Mayor and Members of City Council FROM: Curtis E. Hawk, City Manager SUBJECT: Agenda Item Comments and Other Items of Interest for City Council Meeting July 22, 1997 1. Work session items. Note that in addition to going through the regular agenda items, we have set aside time to discuss regulations for senior independent living units. This item is placed on the agenda at the Council's direction following the completion of Ordinance 480-Y ("Personal Care Facilities"), and picks up where 480-Y left off. It affords senior adults the opportunity to continue to reside in the community with their peers. but in a more convenient and secure environment than perhaps their single family home could provide. Residents in these housing units must comply with the restrictions set forth in the Fair Housing Act. Staff followed up with the Department of Human Services regarding whether or not a spouse or other person not requiring personal care assistance could reside at a personal care facility with an individual who did require such assistance. It seems that the written licensing requirements are silent in this matter, but the agency's policy is to permit such persons to reside in the facility as long as the "unassisted" person is counted in the census for the personal care facility. Honorable Mayor and Members of City Council Agenda Item Comments and Other Items of Interest July 18, 1997 Page 2 Also during the work session, Shelli Siemer, Senior Community Services Coordinator, will make a brief report on the organizational study we are kicking off in the Parks and Recreation Department. 2. Agenda Item No. 5A. Resolution No. 97-52, adopting the City of El Fuerte, Sinaloa, Mexico, as a Sister City. Southlake Sister Cities, Inc. has been discussing the idea of developing a "Friendship Agreement" with another international city for many months. One of the most promising opportunities lies with El Fuerte, Mexico, and the Board of Directors recently voted to send representatives there to further discuss the possibilities. Your packet contains written material from Southlake Sister Cities, describing El Fuerte. Also included is a resolution stating the city's intent to participate in a relationship with this city. Southlake Sister Cities. Inc. respectfully requests your approval of this resolution. 3. Agenda Item No. 5B. Request approval for Councilmember Scott Martin to travel to Mexico for Southlake Sister Cities. Your packet contains the request form as required by your procedures. Councilman Martin, who has been a member of Sister Cities for some time, has expressed an interest in making this trip as a representative of the city. I personally appreciate his willingness to go. and recommend your approval of his request. 4. Agenda Item No. 5C. Authorize the City Manager to enter into a professional services agreement with Appraisal Services, Inc., to appraise eight properties for S.H. 114 takings for the West Dove Street Interchange. As noted in the attached memo from Public Works Director Bob Whitehead, it is our responsibility to appraise and acquire the necessary additional ROW needed by the State to widen S.H. 114. As you are probably aware, Honorable Mayor and Members of City Council Agenda Item Comments and Other Items of Interest July 18, 1997 Page 3 Maguire Partners and Westerra-Timarron have offered to donate additional ROW on tracts of land they own. These appraisals will be conducted on the parcels owned by other landowners. As noted in the staff memo, the appraiser recommends using the "short form" appraisal where possible. It is our intent to do this, but if condemnation becomes necessary, we will have the appraiser do the long form appraisal. Costs could vary due to the difference between the short form and the long form appraisal. The maximum cost to us, assuming all of the appraisals end up being long forms, would be $16,150. The attorneys are reviewing the agreement. If you have any further questions, please feel free to contact Bob. 5. Agenda Item No. 5D. Authorize the City Manager to enter into an agreement with Newman, Jackson. Bieberstein. Inc.. for Southlake Corridor Administration. Design and Landscape Architectural Services, Phase I. This item is a request to execute a letter of agreement with Newman. Jackson. Bieberstein for scheduling, preliminary budgeting, and preliminary design work in conjunction with $10,000 approved in the budget for corridor enhancement projects. In Phase I, the consultant will prioritize and draw up preliminary budgets for proposed future corridor projects. They will also provide a concept plan for Phase I implementation of landscaping. beaming. and minor hardscaping in the state ROW adjacent to Myers Meadow Subdivision. If you have any questions concerning this request. please feel free to contact Greg Last or Chris Carpenter. 6. Agenda Item No. 5E. Authorize the City Manager to enter into a professional services agreement with Glass and Associates for development of a comprehensive strategic plan. We have discussed preparing a strategic plan for the last two budget years, but have not been able to find time to undertake this project. With the completion of our citizen Honorable Mayor and Members of City Council Agenda Item Comments and Other Items of Interest July 18, 1997 Page 4 satisfaction survey, we need to move forward with the project. As you know, Mayor Stacy, Ralph Evans and David Harris have agreed to serve as the Council committee for this project. This item has been placed on your agenda to approve the contract with the consultants, which is attached for your review. Also included is a brief questionnaire to complete. The consultants want input from all Council members prior to beginning work with the committee. We encourage you to complete the enclosed questionnaire and forward it to the consultant in the envelope provided. 7. Agenda Item No. 7A. ZA 97-065. Development Plan for Timarron Addition, Wyndsor Creek, Phase II, approximately 15.925 acres. This item and the following item were tabled from the July 15. 1997 City Council meeting to the July 22. 1997 City Council meeting to allow the applicant time to reconfigure the roadway through the site in cooperation with some of the adjacent home owners. City Engineer Ron Harper has discussed the roadway reconfiguration with Timarron and sees no engineering issues with it. However, the cul-de- sac length and the number of lots on the cul-de-sac exceed the maximum criteria allowed by the Subdivision Ordinance, as noted in the staff review. The Planning and Zoning Commission recommended approval of the new configuration presented at the meeting (4-2- 1) with amendments as noted in the staff report. Commissioner Edmondson and Commissioner LeVrier were the dissenting votes, with Chairman Wright abstaining. Commissioner Edmondson stated that her concern was with the proximity of the development to Metro Pool Chemical. Feel free to contact Dennis Killough if you have any questions concerning this request. 8. Agenda Item No. 8A. ZA 97-066. Preliminary Plat for Timarron Addition, Wyndsor Creek, Phase II. In addition to the above issues, adequacy and manner of the right-of way dedication required for old South Carroll Avenue was discussed at the Planning and r Honorable Mayor and Members of City Council Agenda Item Comments and Other Items of Interest July 18, 1997 Page 5 Zoning Commission meeting. The Commission directed the city staff to determine whether more or less ROW was required and recommended that any ROW dedication be measured from the existing roadway centerline rather than from the existing east ROW line (previously dedicated by the Versailles development). The staff has determined that 64' feet instead of 60' of ROW is required in order to accommodate up to a three lane future roadway along with a 6' on-street trail as shown on the Trail System Master Plan. The applicant has shown a 64' dedication. The Planning and Zoning Commission recommended approval of the new configuration presented at the meeting (5-1-1), with amendments as noted in the staff report. Commissioner Edmondson was the dissenting vote. with Chairman Wright abstaining. Feel free to contact Dennis Killough if you have any questions concerning this request. OTHER ITEMS OF INTEREST 9. Westlake Bankruptcy Proceedings Update. Final arguments were presented on Wednesday, July 16, in Judge McGuire's Court. The Judge took the ruling under advisement and said he will contact the lawyers once he has prepared his opinion. As soon as we are contacted with his judgement we will notify each of you. Although we are uncertain of which way he will rule, the lawyers agree that even if the fraudulent transfer argument is permitted in the State Court. it may not work in Westlake's favor. As always. we will bring, you up to date on the legal issues during Executive Session. 10. Commercial Garbage Collection. Note the attached letter from Dynamic Travel regarding the increase in commercial garbage collection costs. As the response letter indicates, we are aware that we should have notified the businesses or ensured that Laidlaw would provide such notification. We are exploring, options, such as using the Economic Development Newsletter. to inform the business community about such changes in the future. Bob Jones Park Groundbreaking Plans, Kim Lenoir, Director of Parks and Recreation, has been making plans for a groundbreaking ceremony at Bob Jones Park. It is scheduled for Friday. August 8 at 9:00 a.m. at the site. We hope you can all be there. 12. Status of Diamond Circle Sewer Easement Negotiations. Dee Ekstrom is working on getting the easements signed, per our new approach. We anticipate going out for bids soon, with the bid opening scheduled for August 14. 13. Ground Storage Tank Progress. The dedication event has not yet been scheduled, but we are tentatively looking at the week of August 4. TU Electric is to turn on power to the equipment today. There was a leak discovered on the bypass valve at the tie-in to the 30" main. That problem should be resolved by early next week and we will then begin filling the tank to chlorinate and disinfect it. after which water samples must be sent to the health department. 14. Update on Marcus Cable Acquisition of Harron. We were notified this week that Marcus Cable Associates, L.P. completed its acquisition of cable television assets from Harron Communications on July 8, 1997. Under the terms of Ordinance No. 675, the amendment to our franchise agreement with Marcus Cable, the franchise was transferred to Marcus Cable concurrent with the closing.. Harron representatives have informed staff that its customers ‘vill receive information on the transfer in the next billing cycle (August I). All of Harron's customer service and corporate telephone numbers will remain in service. 15. Traffic Accident at S.H. 114 and White Chapel involving City vehicle. As you may be aware, yesterday one of our Public Works vehicles was involved in a traffic accident Honorable Mayor and Members of City Council Agenda Item Comments and Other Items of Interest July 18, 1997 Page 7 with a cement mixer. The City vehicle was south bound on White Chapel and the cement truck was west bound on S.H. 114. The city vehicle (which had the green light) was clipped on the rear by the cement truck and spun around. The cement mixer ended up in the median on its side. Luckily, no one was injured. 16. FM 1709 Water Main work to begin. The final easement has been acquired and construction in scheduled to begin the week of August 10th on the 20" water main on FM 1709. The main will connect the water line at White's Chapel to the future elevated water tank in the Miron Addition. This tank will be connected to the other water tanks as part of the unified system. 17. Gandy, Zoning Administrator SUBJECT: Discussion Item, Ordinance No. 480-AA, Senior Adult Housing When Affiliated with Personal Care Facilities Attached is the first draft of the above-referenced ordinance that the Council asked that I bring back to them on July 22, 1997 for discussion purposes. This ordinance picks up where 480-Y left off. Again, I would like to emphasize that this ordinance provides for the development of senior adult housing units only when affiliated with a personal care facility and as an ancillary use to that development. Section 1 provides a definition of Senior Adult Housing which permits persons age 55 or older to occupy eighty (80%)percent of the permitted forty (40%) percent of the total permitted units within the personal care project. Section 2 reflects the changes to Specific Use Permit#39 that we discussed during Ordinance No. 480-Y. 0ection 3 amends some of the approval criteria for personal care facilities that we discussed at the final reading of Ordinance No. 480-Y. The following will apply to both personal care housing units and any senior adult housing units developed in conjunction with personal care facilities. The first addition provides for an annual certification process to document the occupants' ages and number of vehicles. The second item adds a provision whereby the Chief Building Official may revoke the facilities certificate of occupancy ("C.O.") if the specified age requirements are not met and further provides that additional parking spaces be provided if the number of cars stored on site exceeds the number of spaces available on site. The Development Regulations -- Parking subsection has been amended to accommodate the additional parking spaces needed for senior adult housing units. The residents residing in the senior adult units will likely be more mobile than their counterparts in the personal care units. Therefore, additional parking spaces will be needed to accommodate the additional vehicles. I did follow-up on the question of whether or not a spouse or other person not requiring personal care assistance could reside at a personal care facility with a spouse who did require such assistance. Attached is a copy of my letter to the Texas Department of Human Services and the response of the Licensing Manager. Note the second paragraph of my letter which quoted the definition of resident and the function of a personal care facility from the licensing manual provided by the state agency. Note also that she could not provide a written provision whereby "unassisted" persons could reside at personal care facilities, but instead responded that "historically, the Licensing Section has stated that anyone who is admitted to the facility is considered to be a"resident" and that person may not require "personal care services." •ased on the Licensing Manager's response, it would appear that both spouses could reside in a personal care facility even if only one of them needed assistance with daily care activities. But given these facts, I still feel that Ordinance 480-Y stops short of meeting the needs expressed by the Senior Advisory Committee. It appears that a senior adult whose spouse has died would still be forced to leave the City of Southlake,Texas 0/ community if they chose to sell their single family dwelling and had no family members in the community ho could provide a family quarters as defined by the current zoning ordinance. Assistant City Manager: Shana K.Yelverton In reading the Licensing Standards for Personal Care Facilities which was sent to me ioclate last year, I cannot find the specific cite which would permit the "unassisted" retary: L.LeGrand person in a couple to reside in these facilities. In fact,the definition of resident states- "anyone accepted for care [emphasis added] in the personal care facility"which would seem to indicate that only those persons needing "supervision of or assistance with routine living functions" would be permitted to reside in such facilities. If your response concurs with Ms. Hoffmeyer's, please confirm whether or not this provision is extended only to a spouse or if an independent or a dependent child(ren) as well as a brother or sister or nephew, etc. could also reside with the person needing care offered by a personal care facility. And to extend this idea one step farther,what would be the maximum number of persons not requiring assistance with daily living activities that would be permitted to reside in a personal care facility. You requested information from • the Licensing Standards for Personal Care Facilities. You indicated in your letter that you have a copy of these minimum licensing requirements. Your questions regarding the different relatives who could be admitted to the facility are not specifically addressed in those regulations. Historically, the Licensing Section has stated that anyone who is admitted to the facility is considered to be a "resident." That person may not require "personal care services." However, the individual would be in a licensed personal care bed. That individual would be subject to the same documentation requirements and regulations as any other "resident." The facility has a set number of licensed beds, and they could never exceed that total number of individuals. There would be no requirement that the individual be a relative of a "resident." Section 4, "Definitions," of Ordinance No. 480, as amended, is hereby amended by adding • the definition for "Senior Adult Housing" thereto to read as follows: L\CTYDOCSORD\DRAFTZONTNG\4SOrU-1 ORD 1 • Senior Adult Housing(affiliated with personal care facility) - Dwelling units (developed in conjunction with, and ancillary to, personal care facilities) specifically designed to meet the needs of ambulatory elderly persons. The residents of these facilities shall be restricted to age 55 or older or to persons meeting the definition of handicapped per the Fair Housing Act. To qualify as Senior Adult Housing, a minimum of 80%of the total permitted units allowed by Section 45.1 (39) [eighty percent (80%) of the permitted forty (40%) of the total units] shall have a household head of 55 years of age or older or be occupied by the surviving member(s) of a household, regardless of age, where the household head meeting the age requirements has died. SECTION 2. Section 45.1 (39), "Specific Use Permits"of Ordinance No. 480, as amended, is hereby amended to read as follows: "39. The City Council may only permit the construction of the following ancillary housing units for senior adults (age 62 and over) for independent living purposes (as defined in this ordinance) when offered in conjunction with on- site personal care facilities or tcd thraough when operated in conjunction with on site home health care agencies licensed by the State of Texas. in-the e Personal care facilities with ancillary housing ALL,except AG, RE,SF-1A,SF-1B, units (such as, zero-lot-line-type or clustered SF-30, SF-20A., SF-20B development) for senior adult independent living purposes [up to, but not exceeding forty percent (40%) of the total permitted housing units], are encouraged to be located within walking distance of shopping areas, medical offices,civic centers, public parks,religious facilities,and other related facilities or may be located in transitional areas adjacent to medium density residential developments when compatible and shall be subject to the requirements set forth in Section 45.9 of this ordinance. Personal care facilities with ancillary housing ALL,except AG, RE, SF-1A,SF-1B, units(such as, independent suites)for senior adult SF-30, SF-20A., SF-20B independent living purposes [up to, but not exceeding forty percent (40%) of the total permitted housing units], are encouraged to be located within walking distance of shopping areas, medical offices, civic centers, public parks, religious facilities,and other related facilities and • shall be subject to the requirements set forth in Section 45.9 of this ordinance. Section 45.9, "SPECIFIC REQUIREMENTS FOR PERSONAL CARE FACILITIES," of Ordinance No. 480, as amended, is hereby amended to read as follows: "General Criteria • Approval of personal care facilities and any ancillary housing units shall be based upon an analysis of the location, the site layout and design features,the adequacy of water, sewer, and other public improvements necessary to support the site,the assurance that the adjoining streets can handle higher volumes of traffic during peak hours of traffic loading without a requirement to divert traffic onto traditional residential streets and the compatibility of the construction with adjacent land uses. • The owner/operator of any project developed under an approved personal care facilities specific use permit shall annually on the anniversary date of the issuance of the Certificate of Occupancy, submit certification to the Administrative Official that the development is in compliance with these regulations. Such certification shall include: a) The ages of the heads of household of each housing unit;b)Documentation that those heads of household, not complying with Item 1 a above, qualify for residence as surviving household • members; and c) The number of cars belonging to occupants of each housing unit. • The Chief Building Official shall revoke the Certificate of Occupancy if any;of the housing units are occupied by heads of household who do not comply with the age requirements previously herein stated for this use. If the total number",.of cars registered to occupants and full time staff members exceeds the total number of parking spaces contained within the development, then the owner/operator shall within 30 days of the date of said certification,submit for approval a revised site plan showing the location of sufficient additional car parking spaces to accommodate all the cars belonging to occupants, and the construction of the same to city standards shall be completed within 120 days of final approval of such site plan. Development Regulations • Parking: Ten (10) spaces plus 1.5 spaces per each 3 beds receiving personal care services plus 1.1 spaces for each senior adult housing unit." SECTION 4. This ordinance shall be cumulative of all provisions of ordinances of the City of • Southlake, Texas, except where the provisions of this ordinance are in direct conflict with the LTTYDOCS ORD\DRAFTZONINGw80AA•L ORD 3 • provisions of such ordinances, in which event the conflicting provisions of such ordinances are hereby repealed. SECTION 5. It is hereby declared to be the intention of the City Council that the phrases, clauses, sentences, paragraphs and sections of this ordinance are severable, and if any phrase, clause, sentence, paragraph or section of this ordinance shall be declared unconstitutional by the valid judgment or decree of any court of competent jurisdiction, such unconstitutionality shall not affect any of the remaining phrases, clauses, sentences, paragraphs and sections of this ordinance, since the same would have been enacted by the City Council without the incorporation in this ordinance of any such unconstitutional phrase, clause, sentence, paragraph or section. SECTION 6. IIIAny person, firm or corporation who violates, disobeys, omits, neglects or refuses to comply with or who resists the enforcement of any of the provisions of this ordinance shall be fined not more than Two Thousand Dollars ($2,000.00) for each offense. Each day that a violation is permitted to exist shall constitute a separate offense. SECTION 7. All rights and remedies of the City of Southlake are expressly saved as to any and all violations of the provisions of Ordinance No. 480, as amended, or any other ordinances affecting zoning yard regulations which have accrued at the time of the effective date of this ordinance; and, as to such accrued violations and all pending litigation, both civil and criminal, whether pending in court or not, under such ordinances, same shall not be affected by this ordinance but may be • prosecuted until final disposition by the courts. SECTION 8. L\CTYDOCS\ORD\DRAFTZONTNG\,480AA-I ORD 4 . • The City Secretary of the City of Southlake is hereby authorized to publish this ordinance in book or pamphlet form for general distribution among the public, and the operative provisions of this ordinance as so published shall be admissible in evidence in all courts without further proof than the production thereof. SECTION 9. The City Secretary of the City of Southlake is hereby directed to publish the proposed ordinance or its caption and penalty together with a notice setting out the time and place for a public hearing thereon at least ten (10) days before the second reading of this ordinance, and if this ordinance provides for the imposition of any penalty, fine or forfeiture for any violation of any of its provisions, then the City Secretary shall additionally publish this ordinance or its caption and penalty in the official City newspaper one time within ten days after final passage of this ordinance, • as required by Section 3.13 of the Charter of the City of Southlake. SECTION 10. This ordinance shall be in full force and effect from and after its passage and publication as required by law, and it is so ordained. In short, you are absolutely correct that our commercial garbage customers should have City Manager: received notification of the rate increase once approved by the City Council. Curtis E.Hawk Failure to notify was apparently a classic case of miscommunication (we Assistant city Manager: believed they were going to notify, they stated they were unaware that this was Shang K.Yelverton our expectation). I apologize that you were not formally notified. 4 C Secretary: L.LeGrand I also share your concerns regarding Laidlaw's representatives being unable to respond accurately to your questions regarding the rate increase. My staff contacted Laidlaw, and they now require the customer service representatives to refer these calls to specific staff knowledgeable on the subject. The rates now in place were approved by the Council upon the request of Laidlaw who, incidently, had not requested or received any rate increase since beginning to provide our service in 1992. The new contract was approved after months of negotiation, review by a Council committee, and approval by the full Council. Again, I understand your concerns and will make efforts to work with Laidlaw to ensure that future services changes, rate changes, or other customer issues are handled more professionally. The question I have is why are the patrons not notified when a proposed rate increase is being submitted to the city? Also, when it is approved, why we are not given any advanced notice so we can alter our budget. Your $6 . 34 monthly increase did not alter the budget that much, but if everyone, TU Electric GTE, Water, Lone Star Gas, etc, did the same thing, it could alter it a lot. Southlake Sister Cities has long desired to establish a sister city relationship with a city in Mexico and during the past year have researched various areas for possible cities that might offer mutual benefits for both cities. El Fuerte (pop. 25,000) is a former colonial Mexican town located in the lower Sierra Madre. The downtown architecture is an example of provincial colonial construction with numerous buildings over 200 years old. El Fuerte is located on the world famous Chihuahua al Pacifico railroad(Copper Canyon Train) and is host to many international visitors who start there Copper Canyon visit at El Fuerte. The delegation from Southlake will be hosted by the Prof. Miguel Castillo, Director of International Relations in El Fuerte. A delegation of Southlake area residents and business leaders representing Southlake Sister Cities, Inc., will be visiting the city of El Fuerte, Sinaloa, Mexico, leaving DFW on August 6 returning August 11, 1997. Southlake Sister Cities has wanted to establish a Sister City relationship with a city in Mexico for a long time, and during this past year have researched various areas for possible cities that might offer mutual benefits for both cities. Councilmember Scott Martin has been invited to be a member of the delegation on this trip, representing the Southlake City Council. Scott expressed to me that he has been one of those who have wanted a City in Mexico as a Sister City for a couple of years and has asked for Council approval to attend this trip to Mexico. • According to the Council Travel Policy, Section C (3) Other events and travel expenses, reimbursement for expenses of travel of City Council members for purpose other than those listed in Subsections (c)(1) and (c)(2) must be approved in advance by a vote of a majority of the city council. Dove St. Interchange BACKGROUND As part of agreements made with the Texas Department of Transportation it is the City of Southlake's responsibility to appraise and acquire property for additional ROW for the widening of S.H. 114. The properties that are proposed to be appraised are located adjacent to the proposed S.H. 114-Dove St. interchange. FUNDING/FISCAL IMPACT III The maximum cost for the appraisals is $16,150.00. This is assuming that the appraisals are done in long form. Kenneth Huffman has stated in the attached agreement that it will not be necessary to do all of the appraisals in long form. The attached agreement outlines which properties will be in long form. It also provides prices for both forms for each property. Per our recent conversation, site visit and information sent to us on June 19, 1997, we understand that the following landscape architectural services are requested on the approved Southlake Corridor Study. Specifically the professional services requested are as follows: 1. Preparation of a general recommendation of schedule of priorities for development of enhancements in the recently adopted Corridor Study. The schedule shall be general in content, covering a"ranking of projects to be implemented" based on maximum visual impact on the Corridor area and adjacent private properties. 2. Preparation of general budgets for the approved enhancement Corridor features. Budgets and costs will be expressed in a general "order of magnitude" amount and are only intended to assist the City in determining a general range of construction costs for major Corridor features. 3. Development of a concept level landscape plan which will in the future, serve as an "pilot project" example of how the FM 1709 corridor can be developed. The pilot project area is located on the northern side of FM 1709 at either side of Meadowlark Lane at the entry of Myers Meadow Subdivision in Southlake, Texas. • The concept plan shall be at approximately 1"=30' or 1"=40" scale illustrating walks, sitting areas, shade and ornamental tree plant masses, potential buffering, and concept grading. Plan will generally show location of FM 1709 curb, ROW • line and a general idea of the landscape treatment along the linear greenbelt of Myers Meadow Subdivision. We shall provide one (1) colored plan for your use in presenting the ideas to City P & Z , Council and citizen groups. We anticipate two (2) meetings with you and your staff to present the concepts, get your input and finalize the overall plan. We propose to accomplish the above work on an hourly basis per the attached rates, not to exceed $10,000. Reimbursable expenses are in addition to professional fees and shall be charged to you at our cost. I have attached a copy of the contract submitted by Glass and Associates for this project, which clearly articulates the proposed approach and purpose of this planning process. Note that the cost of the project will be $12,000. • Glass and Associates recently completed the city's second citizen satisfaction survey and a second business survey for the Office of Economic Development. The experience and knowledge they have gained through these projects, as well as the information contained in the survey final reports will be useful during the strategic planning process. Also included in your packet is a questionnaire the consultant is hoping each Council member will complete and return directly to the consultant.
