Item 8 - Moayyad Medical S T A F F R E P O R T March 2, 2012 CASE NO: ZA12-009 PROJECT: Moayyad Medical Buildings EXECUTIVE SUMMARY: Blake Architects, on behalf of Edward Moayyad, is requesting approval of a Zoning Change and Site Plan from “CS” Community Service District to “S-P-1” Detailed Site Plan District for the purpose of adding 950 square feet of floor area to the existing 2366 square foot single story building and adding an additional 3,700 square foot single story building SPIN Neighborhood #10. REQUEST DETAILS: Blake Architects, on behalf of Edward Moayyad, is requesting approval of a Zoning Change from “CS” Community Service District to “S-P-1” Detailed Site Plan District, the proposed zoning regulations would allow a reduced setback along the south property line to 19’ and allow “O1” Office uses to include “Health and Physical Fitness Centers and Gymnasiums” and Site Plan to expand an existing structure and propose a future Phase 2 building consisting of the following: SITE DATA INFORMATION  Existing Floor Area 2.366 sq. ft. Granberry NO. 581 Addition LAND USE CATEGORY: Office Commercial CURRENT ZONING: “CS” Community Service Zoning District HISTORY: - A special use permit for a church use was approved with the passage of Ordinance No. 261-44 in December of 1982. - A plat showing for Lot 4, Hiram Granberry No. 581 Addition was approved in February 1994 and filed in the County in March of 1994. - The “CS” community service district zoning was placed on the property with the adoption of the Comprehensive Zoning Ordinance No. 480 and the corresponding official zoning map in September of 1989. - A request for a Zoning Change and Site Plan (ZA04-020) was brought before the P&Z Commission on May 6, 2004 and was tabled (6-0) until July 8, 2004. On July 8, 2004 it was tabled on Consent (7-0) until August 19, 2004.On August 19, 2004 the request was tabled indefinitely on consent (7-0) on request of the applicant. CITIZEN INPUT: A SPIN meeting was held on February 9, 2012 at Southlake Town Hall and the SPIN Report is attached below in attachment D. SOUTHLAKE 2025 PLAN: Consolidated Land Use Plan Under the Southlake 2025 Consolidated Land Use Plan the subject property is designated as Office Commercial land use designations. Pathways Master Plan & Sidewalk Plan The Pathways Plan indicates an 8-foot sidewalk at the front of the subject property along S. White Chapel Blvd. This sidewalk is proposed to be installed as a part of Phase 1 of the proposed project. TREE PRESERVATION: The applicant is required to preserve at least 70% of the existing tree cover on site with this request and has met this requirement. White Chapel Boulevard Development Details The applicant is requesting a zoning change and site plan for medical office buildings. The current zoning is CS – Community Service District; the proposed zoning is O-1 – Office District. The current land use designation is Office Commercial which is consistent with this zoning request. There is currently one existing building that is one-story and approximately 2,366 square feet. The applicant is proposing to add 950 square feet on the west side. The request also includes a Phase 2 building that is one-story and approximately 3,700 square feet. The following plans were presented at SPIN: / / QUESTIONS / CONCERNS No one outside of city staff, SPIN rep, applicant and property owners attended the meeting, however, two people came in after the meeting adjourned and talked briefly with the applicant. The following changes are needed with regard to driveways: The minimum driveway centerline spacing along N. White Chapel is 250 feet. This plan proposes use of an existing driveway which is approximately 70 feet from an existing driveway approach accessing the undeveloped property to the north. (Variance requested) The minimum stacking depth required is 75’. The minimum stacking depth provided is approximately 30 feet (Variance requested) Provide a common access easement covering the full width of driveway from N. White Chapel Boulevard and to the connecting access easement stubbing into the north boundary from the adjacent property to the north. Provide the documentation of the filing of the easement prior to issuance of a building permit. Please verify that the elevations meet the following design guidelines. “Stucco or plaster shall only be allowed when applied using a 3-step process over diamond metal lath mesh to a 7/8th inch thickness or by other processes producing comparable cement stucco finish with equal or greater strength and durability specifications.” Label the type of masonry to be used on the dumpster. Masonry must match the principal building materials. Please update the variance letter and provide a complete S-P-1 zoning narrative and document describing the permitted uses and development regulations to include the proposed building setback along with any other deviations from the City of Southlake zoning ordinance. The F1 bufferyard and screening regulations require a minimum 8’ fence meeting requirements of Ord. 480 Section 39 as amended. Show and label the type and height of fence, noting whether existing or proposed. Where a fence meeting the requirements exists on the adjacent residential property and is in good condition staff recommends executing an agreement with the adjacent properties for maintenance in perpetuity. Color code landscape plan specific to each plant type. 2. The proposed building additions are over 5,000 square feet so the entire interior landscape and required bufferyards must be brought into compliance with the most current regulation of the Landscape Ordinance and Bufferyards section of the Zoning Ordinance. 3. There is a comment on the Site Plan that a solid landscape screening using 8’-0” tall Holly, will be installed within the south bufferyard. I am assuming that this means the Hollies will be a minimum of 8’-0” when planted? QUALIFICATION TO PREPARE PLANS: For all lots greater than 30,000 square feet, Landscape Plans shall be prepared by a Registered Landscape Architect. For lots less than 30,000 square feet, a Landscape Designer or Landscape Contractor, knowledgeable in plant materials and landscape design may also prepare the landscape plan. Irrigation plans shall be prepared by a Licensed Irrigator or Landscape Architect. The Landscape Administrator may reject plans if deemed of insufficient quality or completeness and require that plans be prepared by a Registered Landscape Architect or other qualified professional. OTHER COMMENTS: 1. Please ensure that the proposed sidewalk/trail matches and connects into the existing sidewalks/trail.  Informational Comments: * SPIN Meeting held January 9, 2012.
