BHB Engineering Inc., representing the Century Property Partners, LLC., is seeking approval of Zoning Change and Site Plan for an existing 5,833 square foot medical and professional
office building to allow an ambulatory surgery center and to modify the site plan to add an awning for loading and unloading of patients, generator enclosure and additional HVAC units
per State requirements. The property is located at 3075 W. Southlake Blvd.

The current zoning is “SP-2” Generalized Site Plan with “O-1” Office District Uses. Mention modification of the regulations as a result of the rezoning.

This is the previously approved site plan approved under ZA06-153. All buildings are approximately 5, 833 sf each. Four buildings have been constructed to date and outlined in blue.
The two buildings adjacent to W. Southlake Blvd. have not been constructed. The proposed ambulatory surgery center is highlighted in yellow.

In the previously approved site plan (ZA06-153), the approved required parking for 5 medical
office buildings and 1 general office building was 214 parking spaces. The development has shared parking throughout the site. In the proposed site plan (ZA16-099), the number of parking
spaces required for the ambulatory surgery center is 1 parking space per bed (8 beds in the facility) and 1 parking space per 4 employees (20 employees will be needed for the facility)
reducing the total number of required parking spaces to 202 parking spaces.

HVAC Units-7
The proposed site plan includes modifications to the building for a required patient drop-off/pick-up awning on the east side of the existing building. A require generator and enclosure
is proposed on the south side of the building. The generator enclosure and awning will match the existing masonry on the existing building and trash enclosure. A concrete pad for 7
HVAC units required for the facility operation is proposed to be located on the south side of the building. Six of the HVAC units will be needed for the facility operation and state
requirements and one HVAC unit will provide fresh air for the building. As a result of the generator site and the HVAC pad site the impervious coverage will increase slightly to 59.8%.

This is the proposed generator enclosure which will be constructed adjacent to the existing trash enclosure. The trash enclosure is approximately 25’-9” x 12’-10” x 8’-4” in size. It
will match the masonry of the existing trash enclosure.
