My name is Kristen Duwe Bonola. I am an owner of Next
Century Dental, Implants & Orthodontics and we have been in
Southlake for over 12 years. I am also the General Partner that
owns a house on Jellico Circle in Southlake and 3 parcels of
commercial land at and adjacent to our dental office, one of
which is the almost 6 acre site proposed today by Mainstreet
Rehab Resort.
We purchased the 8+ acres along the Hwyl 14 frontage as an
investment and also so that my husband and I could ensure
high quality neighbors next door to our livelihood, our dental
business. We have been very selective over the 12 years about
what is developed next door and have not entertained some

contractual offers as a result. We have concluded that the
proposed Rehab Resort is the very best use of the land and
location; it's high-end quality affirms and enhances the
Southlake Community.
This entire area, including the almost 6 acres, was zoned C-3
General Commercial District 27 years ago in September 1989. I
have reviewed the list of people in opposition to this project
and only two families were deeded prior to that time. If those
two families were opposed to the C-3 Commercial District
zoning on this 114 frontage property, they needed to fight the
zoningearly in 1989. All the other homeowners on the list were
deeded after this property was zoned C-3 Commercial and
they must have known prior to their home purchase that this lot
and Hwy frontage area would be developed since it had
been zoned C-3 in 1989. (Attached is the list of homeowners
opposed to the development of this property and their
respective deed date taken from TAD.org)
We purchased this piece of property in 2003 since it was
already zoned C-3 and we platted it the same year. We have
paid $1 million in property taxes since that time, based on its

Commercial C3 zoning.
I have been surprised about the opposition to this project,
especially since the C3 zoning instated in 1989 allows for a
myriad of Commercial applications. The Rehab Resort would
be beneficial for Southlake and a perfect fit for this location;
half way between Forest Park Medical Center and
Grapevine's Baylor, Scott & White and will serve patients in
tandem with Forest Park and Baylor, it will not compete.
Currently, those hospitals send their post-surgical Rehab
patients to Corinth or Bedford.
I understand that change is difficult but the reality is this
property will be developed and it has been known since 1989. I

have been a neighbor for 12 years and will continue to be for
the next 15 so I have tried to truly listen, consider and explore
the neighbors' opposition to this project. As a result of the
same, and in a good faith, Mainstreet hosted a meeting
Tuesday Jan 29th to inform residents about the changes that
had been made by Mainstreet to better accommodate the -
neighbors and we had'.hoped to garner some of the neighbors
support. That night, the number one complaint was still they
did not want trees removed, some citing it would affect their
views. One person expressed that they thought the building
was too big for the lot. Another expressed that when 114 was
built into the current highway, it increased the traffic noise
substantially in the Shady Lane area and they were concerned
that any development on this 6 acre site would increase the
noise even more. A neighbor also cited noise from
"ambulances arriving and departing from Mainstreet all hours
of the night a problem". Medical Waste and trash service was
cited as an issue. Several neighbors expressed that there were
current drainage problems on Shady Lane running off the
undeveloped land and gravel road and they had concern
that development would exasperate that flooding problem.
On January 6, it was discovered that the wrong tree ordinance
had been being applied the preceding 8 months and Tree
Ordinance 585B was the actual appropriate ordinance.
Mainstreet is no longer asking for any variances, including a
tree variance.
Mainstreet has reduced the size of the building more than 6000
square feet and it has now been reduced to 66,000. The size of
the building first floor footprint is only 17% of the lot, well under
the 50 % maximum allowed by the City of Southlake to build.
Southlake Building rules allow 75% impervious coverage with
25% open space - Mainstreet's site plan has 52% of impervious
coverage, 23% less than what is allowed. The size of the

numbers of beds have also:been reduced from 102 to 88 -
more than a 13% reduction
The traffic that will impact the Shady Lane neighborhood by.
the Mainstreet Rehab is minimal. All trafficgenerated from the
Mainstreet Rehab has been rerouted to the existing shared .
exits on the West and East side of Next Century Dental,
Implants & Orthodontics.
I was surprised by the neighbor's opposition to the project on
the basis of additional noise being generated by the Rehab
Resort since they were already upset about the noise
generated by the Hwy 114 traffic. Sound Waves propagate
through air and trees much more so than a building. I was
curious about the expressed Hwy 114 noise and multiple times
drove down Shady Lane to listen for myself. There was definite
Hwy 114 traffic noise at various times of the day and night.
When I was behind the 6 ac lot with only the trees between
Hwy 114 and myself, the Hwy traffic noise was very loud.
However, when I was behind the strip center next door that
houses BBQ Outfitters and Coker Floor Co. or at the houses
north of the strip center, the Hwy 114 traffic noise was non-
existent or minimalized. Physics dictates that the construction
of a 2 story building would serve as a buffer from the Hwy 114
traffic noise. The Mainstreet Rehab Resort would serve as that
buffer to the neighborhood from the Hwy 114 noise. The US
Dept of Transportation and Federal Highway Administration
actually have published materials on their websites about

I knew that ambulances of any kind, including those with
illuminated lights or sirens, are not utilized to transport non-
emergency patients, it is too expensive to do so. However, as a
fact checker, I contacted the Discharge Administrator at
Forest Park Medical Center to obtain objective facts. Susan
Threkheld has been at FPMC since its opening 3 years ago, she
been a Discharge Administrator who coordinates post-surgical
rehab transfers for years and has been a nurse for 45 years. She
is very knowledgeable, seasoned and well versed in the
information she offered. She informed me that non-emergency
patients are only transported to new facilities by Care Vans
that do not have sirens or lights (see pictures below). She also
informed me that it is routine in her industry to discharge
patients being transported to Rehab Facilities between 10am-
2pm, 3pm at the latest and the standard in the industry would
be for Mainstreet Rehab Resort to receive patients between
similar times. Mrs. Threkheld indicated that those discharge
times are purposeful and centered around meals, traffic and
new patients to be settled in to their new destination before
5:30 dinner time.

Another neighbor expressed concern about the Mainstreet
Rehab Resort was about medical waste, extra trash and the
retrieval of those items being all hours of the morning and night
since the Rehab Resort does have patients 24 hours. At Next
Century Dental, Implants & Orthodontics, as well as all other
medical related or dental offices throughout Southlake,
medical waste and bio-hazard materials are bi-products of all
health professions. Every doctor and dental office in Southlake
must deal with medical and biohazard waste. The disposal of
the regulated medical waste is intricate and heavily regulated
by OSHA, DOT, EPA & the FDA. Great care is taken by the
professional medically related staffs and the Biohazard
Removal Companies. Additionally, even though the Rehab
Resort will have residents in 24 hour periods, the Biohazard
Removal Companies work typical business hours and do not
pick up materials in the middle of the night or at 4-5am as the
expressed concern by the Shady Lane neighbors. Republic
Waste Management that empties the Southlake dumpsters,
also works typical business hours 7 am - 6pm and our dumpster
next door is dumped Thursdays usually around 10am.
Finally, several families expressed concern about the current
flooding problem on Shady Lane that is exasperated during
rainy seasons. Developing this 6 acre lot will alleviate any water
run-off from the undeveloped commercial land and will be
remedied with the engineering associated with Mainstreet's

Rehab Resort Construction.
I have spoken with a few of the neighbors this week and have
been informed that a couple of the neighbors hosted a
meeting this past week soliciting and instigating opposition to
the Mainstreet Project. .I have learned that those couple of
neighbors are spreading misinformation, innuendo and lies
about the Mainstreet Rehab Resort; I can only assume to incite
anxiety and a greater turnout of opposition. One such lie being
circulated is that Mainstreet is now attempting to add a 15'
parking garage to the project. I am disappointed when
people of this caliber resort to lies in order to sabotage what or
whom they do not like or agree with. Alinsky tactics are
dishonorable, distasteful and in the end hurt the Entire
Southlake Community of good, no Great people. Honesty and
Integrity matter in discourse.
Mainstreet Rehab Resort site plan now complies with the
thousands of pages of all rules and regulations defined by the
City of Southlake and they have spent hundreds of thousands
of dollars to date getting there. There are no more variances
being requested. They have made good faith efforts to save as
many trees possible. They have decreased the size of the
building and are in compliance with the footprint to lot ratio.
The site more than complies with the Open Space rules and
the building has been moved to the West to preserve more
trees. It is aesthetically pleasing, will offer high-paying skilled
jobs, and will pay significant property taxes to the City of
Southlake and Tarrant County. They have gone above and
beyond in their landscaping, planting 5 times the required
landscaping. They have reduced the surface size of the pond
to 4050 sq. ft to save more trees and made it a wet detention
pond with a fountain, also with intent as a beautiful amenity for
the neighbors as the drive down Shady Lane.
